Semaglutide for weight management escalates weekly in steps: 0.25, 0.5, 1.0, 1.7, and 2.4 mg. The 0.25 mg step is a starting dose for tolerance rather than a therapeutic one, and each step is typically held for four weeks before increasing.

The milligram schedule is straightforward. Converting it into units on a syringe is where the mistakes happen, because that number depends entirely on how a given vial was reconstituted.

Prescription medicine. These are labelled escalation steps, reported for reference. They are not dosing guidance. Dose selection, escalation timing, and monitoring are clinical decisions, and compounded or research-grade material sits outside the regulated supply chain.

Note that doses used for type 2 diabetes differ from the weight-management schedule. Reading a chart from one context into the other is a common source of confusion.

Milligrams to units

On a U-100 syringe, 100 units is 1 mL. Units measure volume, not drug — so the conversion needs the concentration.

  1. Concentration: mg in the vial ÷ mL of bacteriostatic water = mg/mL.

  2. Volume: dose ÷ concentration = mL.

  3. Units: mL × 100.

The same dose is a different number of units in every row. 0.5 mg is 20 units, 25 units, or 10 units depending only on how the vial was mixed. This is why "how many units is 0.5 mg" cannot be answered without the concentration — and why the mg/mL belongs written on the vial label.

Watch the small numbers

At 5 mg/mL, a 0.25 mg starting dose is 5 units. That is a small draw on a 1 mL barrel, where a one-unit error is a 20% error in dose.

For the low end of the escalation, a more dilute reconstitution or a smaller-barrel syringe gives a far more forgiving reading. The trade-off is that the top of the range then risks exceeding one barrel — which is the balance to think about before adding water, not after.

Storage

Storage is the same as any reconstituted vial: 2–8 °C, protected from light, never frozen, and bounded by the shorter of peptide stability and the 28-day limit on bacteriostatic water. The bacteriostatic water guide covers the detail.

Frequently asked questions

How many units is 0.25 mg of semaglutide?

At 2.5 mg/mL it is 10 units; at 5 mg/mL it is 5 units; at 2 mg/mL it is 12.5 units. The milligram figure alone does not answer it.

Why is escalation so gradual?

Gastrointestinal effects are dose-related and most pronounced when the dose changes. The slow schedule trades speed for tolerability.
