“How many units is 2.5 mg of tirzepatide” is the most-asked conversion question in this space, and it has no single answer. Units measure volume. Milligrams measure mass. Converting between them requires knowing the concentration — and concentration is set at reconstitution, not by the compound.

Approved tirzepatide does not require this calculation. Mounjaro and Zepbound come as pre-filled pens and single-dose vials at labelled doses — you never convert anything. This page concerns lyophilised research vials, which are not approved for human use.

What a unit is

A U-100 insulin syringe is marked in insulin units, where 100 units equals 1 mL. So one unit is 0.01 mL — a volume, and nothing else.

The word "unit" is borrowed and misleading here. For insulin, one unit corresponds to a defined biological activity. For every other compound drawn into that syringe, the marking means 0.01 mL and nothing more. See the units-to-mL guide.

Same dose. Five different numbers on the syringe. Anyone quoting "2.5 mg is X units" without stating the concentration is giving an answer that is correct only for their own vial.

Choose a concentration that puts your dose between 10 and 50 units. Below about 5 units, the marking error becomes a large fraction of the dose. Above 50, you are using more volume than necessary. Where the concentration comes from

Concentration equals vial mass divided by the volume of bacteriostatic water added. That one division determines every unit count afterwards.

The error that matters

Reading units as milligrams is a hundredfold error, and it is the single most dangerous mistake in this arithmetic. Someone who believes "25 units" means 25 mg, on a vial at 10 mg/mL, has confused a 2.5 mg dose with one ten times the highest labelled maintenance dose.

  1. Label the vial with the concentration, not just the original vial mass.

  2. Write the unit count for your dose on the label too.

  3. Recalculate whenever the vial size or water volume changes. Never reuse a familiar unit count on a new vial.

  4. Check the arithmetic the first time with the calculator rather than mentally.
