Follistatin-344: Releasing the Myostatin Brake, in Theory
Follistatin inhibits myostatin, the brake on muscle growth. The striking images associated with that pathway come from lifelong genetic deficiency — not from inhibiting it in an adult.

Follistatin binds myostatin and blocks it. Myostatin limits muscle growth, so inhibiting it should permit more — which is why this pathway attracts more attention than almost any other in the muscle context.
The images that circulate alongside it are the problem. They come from a specific situation that a vial cannot reproduce.
Not an approved drug. Follistatin-344 has no approved human indication in the US, EU, or UK and no established human dose. It is sold for laboratory research only, and nothing here is dosing guidance.
Where the dramatic pictures come from
The famous examples — Belgian Blue cattle, whippets with unusual musculature, the rare documented human cases — are all instances of myostatin-pathway deficiency present from conception and throughout development.
Muscle develops over years, and a signal absent during that entire process produces a different outcome from a signal partially blocked in an already-developed adult. The pictures illustrate what lifelong absence does. They do not illustrate what an injection does.
This is the central misreading. A developmental phenotype is being used to advertise an adult intervention. Those are different questions, and no result from the first tells you the answer to the second.
What 344 refers to
Follistatin exists in isoforms of differing length. Follistatin-344 is one; follistatin-315 is another, and they differ in how they distribute — one binds more readily to cell surfaces, the other circulates more freely.
Vendors sell "follistatin-344" as a peptide product. Much of the research that generated interest in this pathway used gene therapy delivery rather than injected protein, which is a substantially different intervention from a reconstituted vial.
Follistatin does other things
It is not a muscle-specific molecule. Follistatin binds several members of the TGF-β superfamily — activins as well as myostatin — and those are involved in reproduction, inflammation, and other processes.
A compound described as a "myostatin inhibitor" is being named for one of its interactions. Broad-spectrum inhibition of a signalling family is not a targeted intervention.
What the human evidence supports
There is no human dose-ranging or efficacy trial for injected follistatin as a muscle-growth agent. What the randomised literature does contain is less exciting and more actionable: resistance exercise combined with adequate essential amino acid intake improves the follistatin/myostatin ratio and muscle outcomes.
The best-evidenced way to shift this pathway in humans is training and protein. That is not a rhetorical flourish — it is what the randomised controlled trial in the reference list actually tested.
