Wegovy weight loss chart: STEP and OASIS trial results

In STEP 1, the main 68-week trial in 1,961 adults without diabetes, average weight changed −14.9% with Wegovy 2.4 mg injection vs −2.4% with placebo, as reported in the FDA label. The higher 7.2 mg injection (Wegovy HD) reached −18.8% in the 72-week STEP UP trial, and the once-daily 25 mg tablet −13.6% at 64 weeks in OASIS 4. These are trial averages from different trials and formulations and do not predict any one person’s result.

Sources

Three formulations, separate trials

Wegovy is semaglutide as a once-weekly injection (maintenance 1.7 or 2.4 mg, or up to 7.2 mg as Wegovy HD for adults who tolerated 2.4 mg for at least 4 weeks) and, since 2025-12-22, as a once-daily tablet (25 mg maintenance). Each panel names the formulation and dose studied. Trials differ in population and length, so figures are not compared across panels.

Numbers are the label’s least-squares means for all randomized patients, missing data imputed from retrieved dropouts. STEP 5 is not in the label and comes from its publication.

Heart outcomes: SELECT

In SELECT (17,604 adults aged 45 or older with cardiovascular disease and BMI ≥27, without diabetes, median follow-up 41.8 months), major cardiovascular events occurred in 6.5% on Wegovy vs 8% on placebo (hazard ratio 0.80, 95% CI 0.72 to 0.90). This is the basis of Wegovy’s cardiovascular indication, approved 2024-03-08.

Side effects in the label

In the pooled 2.4 mg trials (up to 68 weeks), nausea was reported in 44% vs 16% on placebo, diarrhea 30% vs 16%, vomiting 24% vs 6% and constipation 24% vs 11%; 6.8% stopped because of adverse reactions vs 3.2%. With 7.2 mg, altered skin sensation (dysesthesia) was reported in 22% vs 6% on 2.4 mg.

Questions

How much weight do people lose on Wegovy?

In STEP 1 the average change at 68 weeks was −14.9% with 2.4 mg vs −2.4% with placebo, per the label. Results vary widely between people and averages do not predict any one person’s.

Is the Wegovy pill as effective as the injection?

They were tested in different trials. The 25 mg tablet reached −13.6% vs −2.4% with placebo over 64 weeks (OASIS 4); the 2.4 mg injection −14.9% vs −2.4% over 68 weeks (STEP 1). No conclusion about relative effect follows from separate trials.

What is Wegovy HD?

The 7.2 mg once-weekly injection, approved 2026-03-19 for adults who tolerated 2.4 mg for at least 4 weeks and need more weight reduction. In STEP UP it reached −18.8% vs −15.5% with 2.4 mg and −3.9% with placebo over 72 weeks.
