Angiotensin II dosage in published studies and labels

These are the doses used for Angiotensin II in FDA labelling and published clinical studies, reported as each source states them, one row per regimen. They describe what was studied or approved for a specific population. They are not a recommendation, and none of them is a dose for any individual.

Angiotensin II has an FDA-approved human dosage only as Giapreza, a hospital infusion to raise blood pressure in adults with septic or other distributive shock. The label starts at 20 ng/kg/min by continuous intravenous infusion, titrated every 5 minutes by up to 15 ng/kg/min, with no more than 80 ng/kg/min in the first 3 hours and no more than 40 ng/kg/min for maintenance.

TitrationStart at 20 ng/kg/min; raise as often as every 5 minutes by up to 15 ng/kg/min to the target blood pressure; do not exceed 80 ng/kg/min in the first 3 hours or 40 ng/kg/min for maintenance; doses as low as 1.25 ng/kg/min may be used. Once shock improves, reduce every 5 to 15 minutes by up to 15 ng/kg/min.

Where sources disagree

  • Angiotensin II is dosed in nanograms per kilogram per minute as a monitored intravenous infusion; in ATHOS-3 it was given on top of background vasopressor therapy. The label covers no other use or route.
  • The label warns of blood clots: in ATHOS-3, arterial and venous thrombotic and thromboembolic events occurred in 13% of patients on Giapreza against 5% on placebo, mainly deep vein thrombosis, and the label calls for clot prevention during treatment.
  • Giapreza is supplied as a ready-made solution (0.5 mg/mL or 2.5 mg/mL) that is diluted in saline to 5,000 or 10,000 ng/mL before infusion; it is not a powder for reconstitution.
