SARMs and peptides are sold by the same vendors, discussed in the same forums, and are chemically unrelated. They differ in what they are made of, how they are taken, how they act, and — most importantly — in what is known about their harms.

MK-677 is the compound that causes most of the confusion. Ibutamoren is sold alongside peptides, is discussed as one, and is not a peptide — it is an orally active small molecule that acts as a growth hormone secretagogue. It is also not a SARM. See the MK-677 guide.

The safety asymmetry

This is the substantive difference, and it runs opposite to what most people assume about oral being gentler than injected.

SARMs have a documented drug-induced liver injury signal. A 2024 analysis in the European Journal of Clinical Pharmacology examined SARM use and related adverse events including drug-induced liver injury from suspected cases, and a 2025 systematic review in the American Journal of Sports Medicine reviewed SARM abuse among athletes. Individual case reports of liver injury with specific SARMs are also published.

The asymmetry is partly real and partly an artefact of attention. SARMs have been studied for harm because they were used widely enough, and orally, to generate hospital presentations. Most research peptides have no documented harm signal because nobody has systematically looked — which is not the same as being safe. Read the SARM data as "harms found" and the peptide data as "not investigated", not as a safety ranking.

Regulatory position

Both sit outside approved medicine, but not identically.

  • FDA has issued public warnings specifically about SARMs in body-building products, citing serious safety concerns including liver injury.

  • SARMs are prohibited in sport under the World Anti-Doping Code, and are a common cause of positive tests.

  • Several peptides were placed in FDA Category 2 for bulk drug substances, closing the compounding route. See the FDA-approved peptides list.

  • Neither category is approved for the uses they are sold for.

  • Both are commonly sold "research use only" — a legal designation, not a quality grade.

Why oral is the harder problem here

SARMs are designed to survive digestion and undergo first-pass hepatic metabolism, which means the liver processes the full oral dose. That is a plausible contributor to the hepatotoxicity signal and is structurally different from an injected peptide, which enters circulation without that first pass.

This inverts the usual intuition. Injection feels more serious than swallowing a capsule, but the route that concentrates exposure in the liver is the oral one.

If comparing them for a goal

  1. They are not substitutes. A SARM acts on the androgen receptor; a growth-hormone secretagogue does not. Choosing between them is not a preference question.

  2. Baseline and follow-up bloodwork is more clearly indicated for SARMs — liver enzymes, lipids, testosterone, LH and FSH.

  3. Approved alternatives exist for several of the goals in question, with labels attached.

  4. Verification applies to both. Products sold as SARMs have repeatedly been found to contain something other than what is claimed. See the testing labs guide.
